The ADDC seminar is the required seminar for those pursuing certification as a CDP Certified Dementia Practitioner and who qualify for CDP certification.

The ADDC seminar is approved for Nurses in most states for 7 CEUs and the information packet will be provided to you in the student handout notebook. Currently, the Dementia seminar is approved for 7 CEUs in the following states; Alabama, Alaska, Arkansas, California, Delaware, Florida, Georgia, Iowa, Kansas, Kentucky, Louisiana, Massachusetts, Michigan, Nebraska, Nevada, New Hampshire, New Jersey, New Mexico, North Carolina, North Dakota, Ohio, Pennsylvania, Rhode Island, South Carolina, Tennessee, Texas, Utah, Virginia, Washington DC, Washington State, and West Virginia. is effective as of 8/4/2017 and is subject to change. The cost of the 7 CEUS is NOT included in the Dementia seminar fee. The information packet can be found in the back of the student handout notebook explaining who to send your check and the CEU packet to AFTER you have completed the Dementia Care seminar. The cost of the CEUS is $4.00 per CEU or $28.00 which is subject to change by the CEU Network. The CEUs are provided by the CEU Network. Please check with the instructor to confirm which states and which governing bodies their Alzheimer's Disease and Dementia Seminar has been approved if at all for CEU's. It is the responsibility of each instructor to submit the course for CEU approval to different governing bodies. Registration Information
The tuition is $195.00 per person. You will receive an email confirmation of your registration. Once payment is received, you are enrolled unless otherwise notified. NCCDP reserves the right to cancel seminars based on insufficient enrollment. Registrants who need to cancel must provide NCCDP with written notice no later than one week prior to the seminar date. Cancellations are refundable less a $75.00 administrative charge. Continuing Education credits issued will be canceled for nonpayment of required fees.
CANCELLATION POLICY
IF NCCDP CANCELS THE CLASS:
The NCCDP reserves the right to cancel the classes even if you have received a confirmation letter due to insufficient enrollment, travel delays due to weather or other conditions, illness, injury, death, or any other unforeseen circumstance and will not be held liable for employee's time lost or travel costs. You will be notified 24 hours prior to the start of the class if the class is canceled. Be sure we have all of your contact information on the registration form such as; cell phone, emergency contact information, work number, email address for work and personal, etc. If due to travel conditions that are not a fault of the NCCDP we may not be able to get in touch with you until the day of the class. You will then be rescheduled for the next class although it will not be held in the city and state you have previously selected. If you cannot attend an upcoming class there is a $75.00 cancellation fee. A refund of the remaining balance will be mailed to you within 60 days of the cancellation if paid by check. If it is a credit card, the balance will be a refund through your credit card.
IF YOU CANCEL:
If you cancel the class, it must be in writing and received via certified mail 30 days prior to the start of the class. There is a $75.00 per person cancellation fee. Once the seminar begins, there are no refunds. We understand that there are special circumstances to the cancellation and we will make every effort to schedule you for the next class which may not be in the city and state that you previously registered. If you paid by check, the refund will be processed and mailed to you within 60 days. If you paid by credit card, the refund will be processed through your credit card within 60 days of the class.
