ASSOCIATION DISCOUNT FORM: $70.00 per person
Technical school, learning institutions, and universities hosting the Alzheimer's disease and Dementia Care seminars.
If a state or national association hires you to present the ADDC seminar, then each attendee will be provided the association discount form signed by you and placed in front of the CDP application located in the student handout notebook. Before you begin your presentation we ask that you open the notebook and show the student the discount form and the CDP application. The discount form must accompany the CDP online application. The student has 30 days from the date of the seminar to submit their CDP application, to honor the $70.00 rate. You would never collect the applications nor the application fee. The student will complete their initial CDP Application online at portalstaging.nccdp.org. The certificate of attendance must list the name of the association, date of the seminar, address and times, student(s) name, your name CADDCT and CADDCT number. If the seminar is CEU approved, the governing body (i.e. NCCAP Provider Number and number of CEUS) information will also be listed on the certificate of attendance. If the association submitted for CEU approval, the association will provide that information to the attendees. You should always notify NCCDP when you are giving out the association discount form. As a reminder, all seminars must be listed on the calendar. At the association conference, you still will provide your own evaluation, sign-in sheet, student handout notebook, and certificate of attendance. Please fax the sign-in sheet with email addresses within 48 hours to NCCDP to 1.973.860.2244. If the association will be marketing this seminar, the NCCDP needs to approve the brochure and wording before the brochure can be marketed.
CORPORATE DISCOUNT FORM: $40.00 pp
If a corporation wants to hire you to present the ADDC seminar, which includes learning institutions, there should be a designated person who will be the Group Administrator responsible for creating the Online Corporate Group account. The fee is $40.00 pp. The applications must be paid for as a group. The certificate of attendance must list the name of the company, date of the seminar, address and time(s), student(s) name, your name CADDCT and CADDCT number. If the seminar is CEU approved, the governing body information (i.e. NCCAP provider number and amount of CEU’s) will also be listed on the certificate of attendance. You should always notify NCCDP when you are giving out the corporate discount form. As a reminder, all seminars must be listed on the calendar. At the corporate training, you will provide your own evaluation, sign-in sheet, student handout notebook, and certificate of attendance. If you are contracted by the corporation and the corporation will be advertising the seminar, NCCDP needs to approve the brochure before it is marketed.
